Requesting Home Visits

Whenever possible, we encourage our patients to come to the surgery, where our clinical team can provide the best care using the equipment and facilities on site. We are aware and appreciate this is not always possible and we are happy to visit patients at home if there is a clear medical need. The GP will assess the medical need and urgency after speaking to the patient or carer. We cannot visit for non-medical reasons such as lack of available transport.

It is very difficult to fit in visits around surgeries and all our other commitments so please help us by calling reception by 10.00am if you think you may need a visit. Also please provide the best contact number (Mobile) if you are unable to get to your landline for the GP to call you back.

We also have the facility for outside agencies to visit you, so please be aware that it might not be the doctor that visits. This could be.

ECP (Emergency Care Practitioner) who have extended training to undertake urgent home visits at the request of GPs as part of the adult community services multidisciplinary team. They undertake physical assessments and look to either set up a treatment plan, with or without a support package, give self-care advice or if necessary, admit to secondary care for acute care.
