Answer a few short questions and our Reception team will get back to you within 1 working day either with an appointment date and time, or with an acknowledgement and confirmation, we will be in touch when we can offer an appointment. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during practice opening hours.
However, if your submission (whether in person, by phone or online) is received after 5.00pm, it is important you are aware that we may not be able to triage your request until the next working day.
Submissions sent on a Friday after 5.00pm may not be triaged until the following Monday.
The forms will not be read:
- Weekends (Saturdays and Sundays) or on bank holidays.
- Monday to Friday between the hours of 6.30pm to 8.00am.

eConsult for Queries
You can now use eConsult for non-medical requests such as sick notes and test results and for medication and other admin queries — no appointment needed.
Available Mon–Fri, 8:00am – 6:30pm.
